Rupture of the perineum is the second leading cause of postpartum hemorrhage after atonic uterus that occurs in almost all labor, rupture of the perineum: the effect of pushing the mother errors. Based on the initial survey conducted of several BPM city of Padang Panjang. BPM "N" is one of the most labor with rupture of the perineum . From the survey results in the observation case, 10 people in BPM "N" on maternal primigrávida. There are seven (70%) of maternal primigravid ruptured perineum with the wrong technique straining and'3 (30%)' of women giving birth there is no rupture perineum, with straining technique correctly. This study aims to determine the relationship of the technique of straining by rupture of the perineum in primigrávida inpartu mother in BPM "N” Padang Panjang.

Survey research method is analytical observation with cross sectional approach. The population in the study were all birth mothers primigravid in BPM "N" Padang Panjang. In this study, sampling was done by using accidental sampling with samples obtained as many as 17 people. Data collection tool is a measuring tool checkh observation list. Data were analyzed using.chi square test with. SPSS computerized system.

Research results obtained on the effect of straining against rupture perieneum techniques. Mothers with straining technique incorrectly, ie 82.4% and a mother who suffered rapture perineum as much as 70.6%. There is a significant relationship between engineering straining by rupture perineum (P = 0.003).

From this study it can be concluded the majority of respondents with one straining techniques and experiencing rupture perineum with P = 0.003. Suggested for health workers, especially midwives to provide counseling and teaches the correct techniques straining at primigrávida pregnant women.
